Assistive Technology

Assistive Technology (AT) empowers people with disabilities to achieve greater independence and enhance their quality of life.

AT helps accomplish tasks that may be challenging without assistance. Contact us for in-person or virtual demonstrations of devices that can assist with:

  • daily living
  • communication
  • mobility
  • vision
  • hearing
  • memory
  • recreational activities

Community Transition Services (CTS)

The CTS Program assists eligible individuals with securing the necessary tools and community support for independent living. To be eligible, an individual must have Medicaid, reside in a nursing home with a desire to live independently, and be able to support independent living financially. The CTS Program operates under a person-centered planning philosophy, an ongoing problem-solving process to help people with disabilities plan for the future.

You will be assigned a Navigator who will help you find and secure all the resources needed to support independent living.

Focus Hope Food Boxes

Shelf-stable food boxes are available for senior citizens who are 60 years old or older, reside in Oakland or Macomb County, and meet the income requirements below:

Annual Income:
Family of 1: $18,954 or below
Family of 2: $25,636 or below
Family of 3: $32,318 or below

Add $6,682 for each additional family member.

Our team of specialists is ready to assist you in finding community resources. We collaborate with community partners to offer referrals for services that meet the needs of our consumers.

Loan Closet

Our Loan Closet has gently used durable medical equipment (DME) that we lend at no cost.

The inventory varies but includes items such as rollators, wheelchairs, bedside commodes, and incontinence supplies.

Donations are welcomed. Items must be dropped off and picked up at our Troy office.

Michigan Medicare/Medicaid Assistance Program (MMAP)

If you receive disability benefits, are getting Social Security, are about to turn 65, or already have Medicare and have questions, our MMAP counselors can assist you with exploring plan options, new Medicare orientation, fraud, error, and abuse, education, and reviewing your plan for eligibility for Extra-Help or the Medicare Savings Program.

Mobile Outreach

Our outreach team travels to your community to provide free information and service referrals on various topics, including advocacy, benefits planning, housing, veterans services, high school transitions, nursing home transitions, transportation, and assistive technology.

Peer Support

Services to help connect consumers with various resources such as Community Mental Health (CMH), Department of Health & Human Services (DHHS), Social Security Administration (SSA), prescription coverage/medical coverage, assisting in locating appropriate housing, advocating with landlord/bank, and resources to modify homes, etc.

Residential Access Mobility Program (RAMP)

Our RAMP program is designed to provide temporary ramps to consumers needing them for no longer than six months.

These loans are for short-term situations such as recovering from an injury or transitioning out of rehab centers.

Stroke Support Group

The Stroke Support Group offers a welcoming environment for stroke survivors and their caregivers to connect, share experiences, and receive mutual support. Along with social interaction, the group incorporates recreational activities to promote physical well-being and relaxation. It provides a space to build friendships, engage in meaningful activities, and support recovery together. Monthly meetings take place at our Clinton Twp office and various community locations.

Personal Assistance Services Reimbursement Employment Program (PASREP)

Are you someone with a disability who needs assistance to work?

PASREP, is a Michigan-based statewide organization that reimburses eligible persons with disabilities for personal assistance costs required to gain or maintain employment. PASREP provides limited reimbursement of personal care assistance costs for persons with disabilities who are employed. These individuals could not continue to work without personal care assistance. PASREP helps individuals maintain their employment, their quality of life, and their independence.

​PASREP helps break the barrier to going back to work by assisting with out-of-pocket personal assistance costs. This can help bridge the gap caused by lost benefits. PASREP recipients receive reimbursements for personal assistance services up to $1,000 per month.
